Players First Basketball

Acknowledgement & Assumption of Risk

I, the undersigned parent/guardian, acknowledge that participation in basketball training, practices, and related activities with Players First Basketball involves inherent risks, including but not limited to physical injury, falls, collisions, and other potential hazards associated with physical activity. I affirm that my child is physically capable of participating in these activities and assume all risks associated with their participation.

Medical Authorisation

In the event of a medical emergency, I authorise Players First Basketball and its staff to obtain necessary medical treatment for my child. I understand that I am responsible for all medical expenses incurred as a result of any injury sustained during participation.

Release of Liability

I hereby release, waive, and discharge Players First Basketball, its coaches, staff, facility operators, and any associated personnel from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage my child may experience while participating in this program.

Code of Conduct

I understand that all participants are expected to follow the rules and guidelines set by Players First Basketball. Any behaviour deemed inappropriate or unsafe may result in removal from the program without a refund.
